First, you need to pay the fee by clicking the PayPal button below. Once your payment has been received, you will be taken to a confirmation page with a link to download your questionnaire for you to print out.

If you do not have a printer or you do not wish to pay by PayPal, you can send a cheque or postal order made payable to Healthy Choices with details of your name and address to:

Diet & Lifestyle Questionnaire, Pulse Health Screening, 46 Gilbert Close, Kempston, Beds, MK42 8RN. You should receive a diet and lifestyle form within a few days.

Once the form has been completed, send to Pulse Health Screening and you should receive an analysis with recommendations a few days later. Please allow up to 28 days in busy periods.
